Specialized Women’s Healthcare is working with Morgan Records Management to provide fast, secure, and accurate copies of your medical records. To request a copy of your records for yourself and/or for your records to be sent to another healthcare provider, you will need to submit a request electronically to Morgan Records Management through one of the following methods:
 

1. Click here to visit Morgan Records Management's patient records request portal. A printable release form will be placed on their website for patients that are not comfortable submitting their information online.

2. Call Morgan Records Management's medical team at 833-888-0061, and they will mail you a form to be completed and mailed back.

3. Email Morgan Records Management's medical team with any questions at: medical@morganrm.com.

There is a nominal processing fee for all medical records requests regardless of who is requesting the records.
